What To Expect At Your First Visit

Your new patient visit will take about an hour and a half. When you arrive, our friendly front-office team will provide you with forms for registration. To save time, you can complete the forms at home by downloading them below. In addition, please bring your insurance card to your appointment so that we can evaluate your benefits. If you have any questions about the forms, your insurance, or our office, please ask – we're here to help!

After the registration forms are submitted we will review your completed paperwork and ask you about the reasons for your visit. If you don't have gum disease, the hygienist will clean your teeth and Dr. Glaser will perform a comprehensive dental exam. For patients who haven't seen a dentist in a while, we may need you to return another day to finish your procedure.

A Glaser Dental cleaning includes a full periodontal charting which we use to track periodontal health over time. A comprehensive exam includes a tooth-by-tooth cavity check, an assessment of the integrity of existing dental work, an evaluation of how well your top and bottom teeth fit together, and an inspection of your gums for signs of periodontal disease.

Steps To Take Before Your First Visit

First you need to schedule a time and date with our office. We will ask for your contact information so we can send a reminder of your appointment prior to the actual date.

Step 2). Fill Out The First Visit Forms And Bring With You On Appointment Day

Please complete a current Health Status Form, General Registration Form, Financial Agreement, and Release of Records Form. Having the forms filled out before your visit will help Dr. Tyler make the most of your time together.

The Release of Records Form is an official request to your previous dentist to transfer your dental records to our office. This will give us a detailed history of the dental care you received.

Note: Dr. Glaser would like the records from your previous provider BEFORE your visit. Please fill out the Release of Records Form and send to your previous dentist at least TWO WEEKS before your first appointment. Having the most recent and accurate treatment history may help reduce the time and/or cost for your initial care. If that is not possible we will request the records before your second visit.
